As a participant of physical activity with JETT Training and

intend to, and will engage in strenuous physical activities. I acknowledge that these
activities involve certain risks. I understand and accept that participating in these
activities, I voluntarily assume these risks. I mean to include in this assumption of risk,
the risk of in jury, no matter how serious.
In consideration of being accepted as a JETT Training participant, I hereby release and
discharge JETT Training, its managers, partners, agents and employees (whether acting
within the scope of their employment or not) from any and all claims, demands or causes
of action relating to or arising from my presence at, or participating in activities related to
JETT Training which may result in injury to me or to my property or in my death. I
intend this release to bind myself, my heirs, executors, assigns, administrators, and
personal representatives.
